How Little Problems Become Big Ones

According to this report, a physical defect in an under-construction LNG facility is being ignored:

I worked at a nuclear power facility which ignored such a problem. In that case, it was a bent shaft on a main coolant pump. The defect probably existed from the beginning and was ignored. Eventually it led to a plant shutdown which cost the company in excess of one billion dollars in lost revenue.

Such defects may be costly to fix in the beginning, but if allowed to fester they ultimately lead to a catastrophe, possible loss of life, and a HUGE waste of money.

But people never learn. They always try to sweep inconvenient facts under the rug.
